Parental/Legal Guardian Consent: (I) (We), the undersigned, parent(s)/legal guardian(s) of a minor(s), do hereby consent to said minor(s) participating in the Broadlands Association, Inc. event. Furthermore, we understand and agree to the following conditions: (1) Participants are expected to be respectful towards other volunteers, staff members, and guests and must follow all safety rules and instructions provided to them. If any participant(s) behaves in a manner that is emotionally or physically harmful to guests, staff, or volunteers at the event, we will be contacted immediately to pick up our participant(s). (2) The HOA IS NOT RESPONSIBLE FOR ANY MINORS WHO LEAVE THE PREMISES WITHOUT PERMISSION. (3) Broadlands Association, Inc. reserves the right to use photos and videos of participants for informational and promotional purposes. However, we understand that our minor(s) will not be identified by name. (4) By completing this form, we are giving express permission for our minor(s) to attend. Parents may not stay at the event without signing up as a chaperone – space is limited for chaperones.
